Lawyer: Man charged with taping toddler's mouth just being 'playful ...

Andre Curry was in the living room playing with his 22-month-old daughter last December when he grabbed a roll of blue painters tape, bound the little girl?s hands and feet together and put two strips of tape over her mouth.

Then the South Side father snapped a photo of the little girl bound in tape and posted it to his Facebook page, according to testimony today at his trial on aggravated domestic battery charges.

Across the top of the image were the words: "This is wut happens wen my baby hits me back," according to testimony. The message was followed by a winking emoticon. Within hours, it had gone viral on the Internet.

Curry, 22, does not deny posting the photo. At the close of the bench trial, which featured only three witnesses and lasted a little over an hour, his attorney, Sam Adam Jr. argued he was a good father who was just being playful, and that the child was never in any danger.

?Was this something stupid to do? No question about it,? Adam told Judge Lawrence Flood. ?But they?re asking you to find him guilty of some very serious charges.?

Cook County prosecutors argued that by placing the tape over the girl?s mouth, Curry obstructed her breathing and committed a battery. There was no evidence presented of any injuries to the child.

Curry?s family, including the baby?s mother, Yasmin Doss, has stood by him since the incident.

His sister, Annastasia Curry, testified today that she was cooking dinner that night at the family?s home in the 6100 block of South Racine Avenue and could hear Curry and her niece laughing in the living room. She testified she went out to check on them at one point and saw the little girl with the tape on her.

?She was laughing, he was laughing,? the sister said in a matter-of-fact tone. ?I saw him take the tape off, and I went back in the kitchen.?

Chicago police Detective Charles Hollendoner testified he interviewed Curry twice in the days after the incident and the father insisted the whole thing was meant to be a joke.

??He said he was just playing with the child and didn?t mean any harm,? Hollendoner said. ?He said the child was only taped up for a maximum of 30 seconds.?

Curry told police he was soon flooded with ?hate mail? and removed the photo from his page the next day, Hollendoner testified. As the image was circulated it also prompted numerous calls to police and the Illinois Department of Children and Family Services.

Curry, who is free on bond and has since taken a job in Minnesota, sat at the defense table today dressed in a cream-colored sweater and black jeans. His attorneys presented no witnesses, and he told the judge he did not wish to testify in his own defense.

After prosecutors concluded their case, Flood acquitted Curry of unlawful restraint, but he said he wanted more time to look over domestic battery laws before issuing a ruling on Nov. 8.

If convicted, Curry could face up to 7 years in prison. He has been ordered to have no contact with his daughter except through DCFS until the case is resolved.

Text, don't call, once Sandy hits, say carriers

Wireless carriers have made extensive preparations to deal with the impact of Hurricane Sandy, and are asking customers to do the same. Among the best things you can do to help keep your phone battery going and to ease network congestion is to limit your voice calls???keep them short???and to send text messages instead.

"Limit non-emergency calls to save battery power and free up wireless networks for emergency workers and operations and send brief text messages instead," Verizon Wireless says in a hurricane preparedness statement. "When the network gets busy, texts have a higher chance of getting through the first time and can be more efficient."

Among other tips offered by Verizon, AT&T,?Sprint and T-Mobile:

  • Keep phones and phone accessories???batteries and chargers???in sealed plastic bags?to avoid water damage.
  • If you have an extra phone battery and you?have power, charge that battery so it's ready.
  • Add to your phone's contact list all key emergency phone numbers and email addresses, including police,?fire and rescue agencies; schools and service people.
  • If you are being evacuated, forward home or work phone calls to your wireless number.
  • Use your phone's camera to take photos or video of your property and valuables before the storm hits, so you have "before" photos if your home suffers storm damage.
  • If cell service is down in your area, but your home Wi-Fi network is working, switch to Wi-Fi on your phone.
  • Expect high-call volume, which can lead to network congestion. If that's the case, you'll hear "fast busy" signals on your wireless phone (or a slow dial tone on your landline phone). If that?happens, hang up, wait several seconds and then try the call again. That allows your original call data to clear the network before you try again.

Scientists decode 'software' instructions of aggressive leukemia cells

ScienceDaily (Oct. 29, 2012) ? A team of national and international researchers, led by Weill Cornell Medical College scientists, have decoded the key "software" instructions that drive three of the most virulent forms of acute lymphoblastic leukemia (ALL). They discovered ALL's "software" is encoded with epigenetic marks, chemical modifications of DNA and surrounding proteins, allowing the research team to identify new potential biomarkers and therapeutic targets.

The research, published in Cancer Discovery, is the first study to show how these three different forms of white blood cell cancer are epigenetically programmed by several different molecules controlling cascading biological networks that manipulate normal gene function, directing cancer development and growth.

"Epigenetic programming is the software that is written on to human DNA, which can be viewed as its hard drive. This programming contains the instructions that determine how cells including leukemia cells function and cause disease," says the study's lead investigator, Dr. Ari Melnick, associate professor of medicine and director of the Raymond and Beverly Sackler Center for Biomedical and Physical Sciences at Weill Cornell Medical College.

"Finding the instructions that ultimately lead to cancer development, and to the especially bad outcome seen in patients with these different forms of ALL, is especially urgent. Epigenetic instructions are contained in many chemical layers. Our study is the first to integrate the decoding of many layers simultaneously, which has enabled us to unlock some of the mysteries explaining the malignant and aggressive behavior of these leukemias," says Dr. Melnick, who is also a hematologist-oncologist at NewYork-Presbyterian Hospital/Weill Cornell Medical Center.

Abnormal Epigenetic Programming Leads to Poor Outcomes

The research team examined abnormalities in the "software" epigenetic programming that leads to the three forms of adult B-acute lymphoblastic leukemia (B-ALL), the most common form of ALL. These forms are BCR-ABL1-positive B-ALL, E2A-PBX1-positive B-ALL, and MLLr-B-ALL. These three B-ALL subtypes feature mutations of different master regulatory genes which force bone marrow cells to produce cancer-promoting proteins. Long-term survival is less than 40 percent among these patients.

"Similar to normal tissue, we believe that tumors may be dependent on specific patterns of epigenetic programming -- especially in B-ALL, where studies suggests epigenomic programming is globally disrupted," Dr. Melnick says. "Our goal was to identify epigenetically modified genes and the molecular machines that cause them to become abnormally programmed."

To that end, the research team performed DNA methylation and gene expression profiling on 215 adult B-ALL patients enrolled in the ECOG E2993 clinical trial, a multi-center and multi-national study, testing different forms of treatment in patients with ALL.

Researchers identified core epigenetic gene signatures that were associated with abnormal fusion proteins. In the case of BCR-ABL1-positive B-ALL, they found that the most deregulated gene network centered around an extraordinarily epigenetically deregulated gene they identified as interleukin-2 receptor alpha, which encodes a protein called CD25.

"Among patients who had BCR-ABL1-positive B-ALL, it was those with aberrant epigenetic programming of CD25 that had significantly worse outcome," says Dr. Melnick. "It's the patients that have this programming glitch that do really poorly."

Although the researchers don't yet know what CD25 does, and why it is important, they say CD25 will be a useful biomarker to test for patients that are at highest risk for poorer outcome.

Dr. Melnick stresses that therapeutic antibodies to the CD25 protein already exist that can, theoretically, destroy leukemic cells expressing this protein. The research team showed that using the CD25 antibody successfully killed BCR-ABL1-positive B-ALL in laboratory experiments. "One could potentially conceive of a human clinical trial where those antibodies are used to attack these cancerous cells," he says.

Researchers also discovered what is writing the bad software in the other two B-ALL subtypes. In both cases, the abnormal cancer proteins E2A-PBX1 and MLLr turn out to be directly involved in altering the epigenetic programming of leukemic cells. Remarkably, MLLr epigenetically turns on a powerful cancer protein called BCL6. In the study, drugs developed by Dr. Melnick that block BCL6 activity potently killed and suppressed the ALL cells in patients enrolled in this clinical trial, which warrants the testing of BCL6 inhibitors in this aggressive form of ALL.

"This study links the direct actions of oncogenic fusion proteins with disruption of epigenetic regulation that leads to abnormal production of cancer-driving genes," Dr. Melnick says. "It potentially provides us with a biomarker for cancer outcomes as well as potential treatments in these aggressive forms of leukemia."

Hurricane Sandy: With 8.1 million in the dark, utility crews get busy

Hurricane Sandy inflicted most of its damage in heavily populated states including New York and New Jersey. The storm also caused three nuclear power plants to shut down.

More than 8.1 million customers in 17 states were left in the dark in the wake of hurricane Sandy, which smashed ashore Monday night in New Jersey and immediately began dismantling chunks of the eastern power grid.

In terms of its outage impact, hurricane Sandy was roughly on a par with hurricane Irene, which in August 2011 knocked out power to more than 9.3 million customers along the East Coast and in Puerto Rico ? about 6 million in 13 East Coast states.

But Sandy's impact was more concentrated than Irene's, doing the bulk of its damage in fewer but more heavily populated states. Most of the outages were in eight states: New Jersey, New York, Pennsylvania, Connecticut, Maryland, Massachusetts, Ohio, and West Virginia ? some 7.4 million customers.

"The magnitude of this event is really huge," says Mark Merritt, president of Witt Associates, a public-safety and crisis-management consulting firm in Washington. "From North Carolina to Maine, utilities are dealing with coastal power issues from flooding to snow impacts in the mountains of Virginia and West Virginia. A lot of these companies, public and private, are competing for limited resources, scrambling, so it's a challenge for everyone."

With crews coming from all over, even the West Coast, hurricane Sandy has become an "an all-hands-on-deck-type event," Mr. Merritt says. Still, there were signs that power restoration could be slow in some of the hardest-hit areas.

Restoration of power in New York, for one, is going to be exceedingly complex, with blown transformers, flooded underground utilities, and waterlogged subways. Repairs may go more slowly than many had hoped, despite the high level of preparation by utility companies.

Excessive flooding from the storm surge knocked out a number of substations in New Jersey?s Essex, Hudson, and Middlesex counties, according to the utility Public Service Electric and Gas (PSE&G). Those stations, which serve about 462,000 customers, will be out of service until the water recedes and equipment can be cleaned and dried, the utility reported.

Consolidated Edison, a big New York utility, said first reports indicated more than 200 wires down on Staten Island. The company cut service overnight to two sections of lower Manhattan and a chunk of Brooklyn to protect underground delivery systems from floodwaters. The electrical equipment there would then have to have all components cleaned of seawater, dried, and tested.

Meanwhile, three nuclear power plants in the northeastern United States had to shut down, and two units cut back their power output as a result of effects from hurricane Sandy, the US Nuclear Regulatory Commission reported.

PSEG Nuclear manually shut down its Salem Unit 1 in southern New Jersey at 1:09 a.m. Monday when four of the station?s six circulating water pumps became unavailable due to weather impacts. Circulating water pumps use Delaware Bay water to condense steam on the nonnuclear side of the plant.

Entergy Nuclear also shut down its Indian Point Unit 3 reactor, 38 miles north of New York City, at 10:45 p.m. on Monday "due to external electrical grid issues," the company reported. About two hours earlier, at 9 p.m., Constellation Energy reported that its Nine Mile Unit 1 near Oswego, N.Y., experienced an automatic reactor shutdown whose cause was being investigated.

Two of six refineries were shut down as of Tuesday morning, with the four others operating at reduced capacity, the Energy Department reported.

At a news conference, New Jersey Gov. Chris Christie (R) spoke of homes knocked off foundations and amusement-park rides rammed into the sea. ?The level of devastation on the Jersey Shore is unthinkable,? he said.

Governor Christie, who has indicated a distinct disinterest in the presidential election, nevertheless praised President Obama on his response to, and preparation for, the storm.

"The president has been all over this, he deserves great credit.... He gave me his number at the White House, told me to call if I needed anything, and he absolutely means it,? he said. ?It's been very good working with the president, and his administration has been coordinating with us great."

According to some early estimates from economists, hurricane Sandy, with its massive flooding, is more akin to hurricane Katrina than hurricane Irene. Katrina was the nation?s most expensive natural disaster costing about $41 billion in property damage, according to one insurance company report.

Now the race begins to see how fast electricity can be restored to limit the overall economic damage. Utilities have been under pressure since major power outages from storms last year to do a better job this time. To that end, they have been prepositioned thousands of line crews from out of state. Now politicians and customers will be waiting ? and waiting ? to see how long it takes.

?We expected an unprecedented storm,? New York City Mayor Michael Bloomberg said at a press conference Tuesday morning. ?That?s what we got.?

Back from the wild, old manatee pals reunite at SeaWorld Orlando

Lil Joe the manatee needs an agent for the unfolding story of his life, which continues to amaze and now includes an unexpected reunion with an old pal.

Lil Joe was plucked recently from a shallow river in central Florida, where he had been marooned and starving for weeks. He was so dehydrated and underweight that the knobs and recesses of his skull were strikingly visible, his ribs were bulging and the loose skin of his concave belly was bunched up.

He was taken back to SeaWorld Orlando, where he had lived as an orphaned newborn, and was reunited with Slip, his longtime aquarium mate. The two manatees, who first met nearly 20 years ago, were kindred spirits during years of touring the nation before they were set free together a little more than two years ago.

Now, when they aren't performing balletic corkscrews in their SeaWorld tank, the reconnected manatees are between them devouring 200 heads of restaurant-grade romaine lettuce each day. And if they aren't thinking about things as humans might, they might at least be feeling a primal tug that amounts to: "What's next?"

Lil Joe was first rescued from the Halifax River near Daytona Beach on July 30, 1989, weeks old and weighing 42 pounds. With his mother presumed dead, Lil Joe would gain fans at SeaWorld as a pudgy, bottle-raised orphan. He was even fed once by President George H.W. Bush.

Slip, named after the marina spaces where boats dock, was born at SeaWorld on Nov. 22, 1991. His mother, Marina, had been rescued in 1979 and was SeaWorld's first bottle-raised calf. She died of complications three weeks after his birth.

The two orphans grew close, perhaps not by choice but as a result of the protocols for rearing captive members of the endangered species.

Biologists thought then that orphaned calves reared in captivity would never acquire the skills needed to survive in the wild. So they sent them off together for years of adventure. First stop: SeaWorld in San Diego. Next, the Cincinnati Zoo & Botanical Garden, where they wowed visitors for four years. It was there that Lil Joe bulked up to a weight of 1,950 pounds, or nearly a ton.

In 2009, the two were shipped to the Lowry Park Zoo in Tampa and introduced to the kinds of river plants that manatees ordinarily eat. The science of manatee care had evolved as Slip and Lil Joe grew, and biologists at that point were confident the two could be released.

So on Feb. 15, 2010, they were set free at Blue Spring on the St. Johns River, where the relatively warm spring water attracts hundreds of manatees each winter. The two had apparently had enough of each other and went their separate ways.

Lil Joe turned up a few months later farther north in the St. Johns, stunned by cold weather. He was rescued, rehabbed and put back into the river several months later. Then, as winter approached last year, he slipped out of his radio-tracking belt and disappeared. Lil Joe was feared dead.

Meanwhile, on Jan. 4, Slip was hauled from Crescent Lake, which connects to the St. Johns River. Stressed by cold, he was taken back to SeaWorld Orlando, about 60 miles to the south.

By then, it seemed the Slip-Lil Joe relationship was no more. But in August, a manatee appeared in the Little Econlockhatchee River ? 25 miles upriver from the St. Johns, a highly unusual place for a sea cow to go.

A few weeks later, a wildlife volunteer spotted an "R 5" brand on the animal's back, confirming it was Lil Joe. (Slip is "R 1.") State biologists suspected that the river had receded after early summer rains, trapping Lil Joe.

Steve Lehr, assistant curator of mammals at SeaWorld, said the manatee was ailing when he was rescued Sept. 27. He tipped the scale at just 1,010 pounds, little more than half his Ohio weight. He was tube-fed water for a few days, then a watery gruel of mashed romaine lettuce and high-protein monkey chow.

The Giants have won two Super Bowls with Tom Coughlin, Eli Manning and Co., and they?ve had three other playoff appearances since Coughlin became the coach and Manning became the franchise quarterback. But they haven?t had a team as good as this one.

This Giants team ? the team that went to Dallas and won one of the wildest games of this NFL season against the Cowboys on Sunday ? is the best Giants team yet, the most complete Giants team yet. This is the Giants team that has the best chance of standing apart and being remembered as not just a good team, but a dominant team.

At this point I can hear you saying that a great team would have put the Cowboys away easily on Sunday, rather than having to hold on to win 29-24 after taking an early 23-0 lead. And you might be right. I?m not here to argue that these Giants are a dominant team along the lines of the ?85 Bears. But the thing is, while the Giants have won two of the last five Super Bowls, they?ve really never dominated: They were 10-6 before going on a run in the postseason and winning the Super Bowl after the 2007 season, and 9-7 before turning it on in the playoffs again last year. Those were mediocre regular-season teams that played their best football in the playoffs.

In the regular season at least, this is a better Giants team. They?re 6-2 and they?ve outscored their opponents by a total of 73 points for the season, a huge improvement from last year, when the Giants were actually outscored by an aggregate score of 400-394 over the course of the season. This year?s Giants are getting big performances from newcomers like running back Andre Brown (who has 236 yards and five touchdowns this season), running back David Wilson (the first-round draft pick who is making a big impact on kickoff returns), tight end Martellus Bennett (who arrived this year as a free agent from Dallas and already has a career-high 334 receiving yards), receiver Rueben Randle (the second-round draft pick who got things started on Sunday with a 56-yard catch on the third play of the game), offensive tackle Sean Locklear (who arrived as a free agent this year and has started all eight games) and strong safety Stevie Brown (who signed as a free agent this year and was their best defensive player on Sunday, with two interceptions and a fumble recovery).

The Giants now lead the NFC East by 2.5 games and are about as close as it gets at the midway point of the season to being a sure thing to win their division. The big question is whether this will be the best Giants team yet in January and, if all goes well, in February. And if there?s anything the Giants have already taught us with the way they?ve played in the playoffs, it?s that being the best in the regular season doesn?t necessarily translate to being the best in the postseason.

But after watching the Giants over the first half of the regular season, would you bet against Coughlin, Manning and Co. in a playoff game? I sure wouldn?t. The defending champions look better than ever.

Here are my other thoughts on Sunday?s NFL action:

I guess Juan Castillo wasn?t the only problem in Philadelphia. Eagles coach Andy Reid fired Castillo as his defensive coordinator because the Eagles blew a lead against the Lions, but judging from the way the Eagles played on Sunday against the Falcons, maybe Reid is the one who deserves to be fired. In their first game with Todd Bowles as defensive coordinator, the Eagles? defense got shredded in a 30-17 loss to the Falcons. Sunday?s game was a great opportunity for the Eagles to prove themselves as contenders, and instead they laid an egg.

What were the Steelers wearing on Sunday? If I tried my best to design the ugliest uniforms imaginable, I wouldn?t be able to come up with anything more hideous than that combination of tan pants and black-and-yellow striped socks that the Steelers tried to pass off as throwbacks on Sunday. Those weren?t NFL uniforms, they were rejects from the discount rack at the Halloween costume store.

Ho-hum, Tom Brady had a huge game. Brady has been so great for so long that it seems like no one even raises an eyebrow anymore when he does something like throw for 304 yards, four touchdowns and no interceptions in a 45-7 win, as he did on Sunday. Brady makes the spectacular look routine, and the Patriots are playing consistently great offense like no other team in NFL history: Sunday?s win was the Patriots? 17th consecutive game with at least 350 yards of total offense, topping the Greatest Show on Turf Rams of 1999-2000 for the longest such streak ever.

DeAngelo Hall needs to knock off the bush league nonsense. Hall, the Redskins? veteran cornerback, was ejected from Sunday?s game against the Steelers after taking off his helmet, getting in an official?s face, screaming at him and refusing to stop after being repeatedly told to go back to his sideline. Hall is in the fourth year of a six-year, $55 million contract. When you?re getting paid that kind of money, is it too much to ask that you conduct yourself like a professional?

It was nice of Matthew Stafford to show up. Finally. Stafford, the Lions quarterback who had a breakout 5,038-yard, 41-touchdown season last year, had seen a sharp statistical decline this season, and for long stretches in some of the Lions? losses Stafford looked like he was sleepwalking on the field. But in Sunday?s win over the Seahawks, Stafford had his best game of the season, throwing for 352 yards and three touchdowns and also running for a score. If Stafford keeps playing like this, don?t count the Lions out of the playoffs just yet.

Stick a fork in the Jets. A week ago I was impressed with the effort the Jets gave in an overtime loss to the Patriots. But there was nothing impressive about the embarrassing way the Jets gave up in Sunday?s 30-9 loss to the Dolphins. At 3-5 the Jets are in last place in the AFC East, and they?re going to be out of the playoffs for the second straight year.

How much fun would Colts-Broncos be in the AFC playoffs? After both the Colts and Broncos won on Sunday, it?s looking entirely possible that we?ll see an AFC wild card playoff game in which Peyton Manning?s Broncos host Andrew Luck?s Indianapolis Colts. What a great story that would be. The only thing that could top that is Peyton facing his brother Eli in the Super Bowl.

Internet Radio Fairness Coalition Forms To Overhaul Net Radio ...

IRFCThe Internet Radio Fairness Coalition, which has been assembled by several Internet and broadcast radio services including Pandora, Clear Channel and many others, officially launched last week. The Coalition comes together with a mission of urging Congress to support legislation of the Internet Radio Fairness Act of 2012 (IRFA), which they believe will bring the royalty system for Internet radio into the 21st century. ?

The Coalition believes that the IRFA will create a sustainable business model for the music industry, giving consumers ?more choices and more products for listening to the music they live; enable artists to earn more money as Internet radio grows; create a marketplace that will attract entrepreneurs to invest in new, innovative ways to deliver music to the public; and drive higher revenues for record labels.?

"Our piecemeal legislation covering music royalty rates was enacted decades before we had the Internet or current technology,? said Ed Black, President & CEO, Computer & Communications Industry Association (CCIA).

The legislation (introduced on Sept. 21) calls for all digital music services to be judged by the same rate-setting standard, which the Coalition feels will put Internet radio under a standard that is balanced for ?the promotion and creation of creative works? and one that ?encourages the growth of robust and sustainable markets for these works.?

The current royalty system, overseen by the Copyright Royalty Board (CRB), splits digital music into two categories of?Internet radio and satellite radio that each carry its own royalty rate-setting standard.

?We believe that market-based solutions are the way to go," said Bob Pittman, CEO of Clear Channel.? "But in the absence of these agreements, the CRB needs to have and consider more relevant information so they are better able to develop a rate structure that will lead to a healthy, sustainable Internet radio marketplace. This will enable artists to earn more and connect more with their fans, consumers to have more choices, and entrepreneurs to invent and invest in new services."

The Coalition believes that the standard for Internet radio is inappropriate and limited in the data that can be considered, and therefore substantially hinders the growth of Internet radio businesses and platforms, as well as hurts consumers, artists and entrepreneurs. ??

?This is a clear case of an out-dated legal framework that stifles technology and consumer options,? said Lee Knife, Executive Director & General Counsel of the Digital Media Association (DiMA). "This bi-partisan solution will drive more innovation in the legal digital distribution of music.?

The IRFA would also:

  • Provide all forms of digital radio the same standard at rate-setting proceedings before the panel of judges on the Copyright Royalty Board;
  • Provide for Presidential nomination and Senate confirmation of CRB judges, and;
  • Promote a competitive marketplace by making it easier for artists and labels to negotiate directly with digital radio services and arrive at a negotiated rate outside the CRB process.

"Legislation that establishes a fair royalty rate setting-standard for Internet radio will drive investment in webcasting, which ultimately offers greater opportunities and more revenue for working artists," said Pandora Founder and Chief Strategy Officer, Tim Westergren. "Internet radio has been shown to help decrease music piracy and increase music sales. When the digital music sector is allowed to grow and innovate, everybody wins."

Calling an ambulance improves heart attack survival

ScienceDaily (Oct. 17, 2012) ? Patients with chest pain symptoms who call an ambulance have quicker, more appropriate treatment and better survival according to research presented at the Acute Cardiac Care Congress 2012. A study from Denmark shows that calling emergency services allows pre-hospital triage and transport to the most appropriate hospital, while a Turkish study reveals that only 29% of patients with chest pain went to hospital by ambulance.

The Acute Cardiac Care Congress 2012 is the first annual meeting of the newly launched Acute Cardiovascular Care Association (ACCA) of the European Society of Cardiology (ESC). It takes place from 20 to 22 October in Istanbul, Turkey, at the Istanbul Lufti Kirdar Convention and Exhibition Centre (ICEC).

Primary percutaneous coronary intervention (pPCI), also called balloon treatment, opens the coronary vessel and is preferred over intravenous (IV) medicine (called thrombolysis) to dissolve blood clots in patients with heart attacks. To deliver this care, patients must be transported to a hospital that can provide balloon treatment. This requires well functioning treatment networks which integrate the pre-hospital and in-hospital phases of patient management so that even patients in remote areas receive high quality treatment that adheres to ESC Clinical Practice Guidelines.

"Transporting patients to hospitals with balloon treatment capacity often involves bypassing local hospitals in the vicinity of the patient," said Dr Mikkel M. Schoos from the University Hospital of Copenhagen, Denmark. "Pre-hospital triage involves digital transmission of the ambulance electrocardiogram (ECG) using telemedicine to the attending cardiologist at the hospital with the balloon capacity who can decide if the patient needs balloon treatment."

He added: "In this way, patients can be referred directly to the hospital that can perform balloon treatment, without first being seen in a local emergency room. This saves important time. We know from previous studies that reduced time to treatment equals greater salvage of the heart tissue near the blood clot after a heart attack."

Dr Schoos' study investigated the quality of these treatment networks in Denmark. The researchers found that 75% of all patients can be transferred directly to hospitals with balloon treatment capacity using pre-hospital triage.

But even when well functioning pre-hospital triage with telemedicine is in place, for 25% of patients it is not possible to deliver treatment within the time targets recommended by ESC Clinical Practice Guidelines if only ground ambulance is used. These are patients who live more than 100km (60 miles) away from the hospital with balloon treatment facilities and patients who go to a local hospital first.

The study showed that patients who are first brought to a local hospital lose important time and this increases their risk of death. "The time delay caused by first being taken to a local hospital that does not have the treatment facilities the patient needs is bad for the patient," said Dr Schoos. "Our study also shows that this system delay predicts all cause mortality in these patients who have big heart attacks. That means that a pit stop at a local hospital increases the risk of death."

There are several reasons why patients might go to the local hospital first. One is that the heart attack is not developed enough to be detected and diagnosed by the ambulance ECG. Dr Schoos is currently investigating ways to improve early diagnosis with ECG or biomarkers in the blood. A second reason could be a failure to transmit the ambulance ECG to the hospital. A third reason is that emergency medical personnel are not confident that the patient is stable enough for further transport and decide to do a pit stop at a local hospital.

Dr Schoos said: "By doing that they postpone the only right treatment. The majority of patients with acute heart attacks can be safely transported to a hospital with the needed treatment capacity, even if these hospitals are further away than smaller local hospitals."

He added: "Even though patients might get arrhythmias, which are bad heart rhythms, as a consequence of their blood clot, these can easily be treated in the ambulance by well educated emergency medical staff."

Ambulance staff must also be educated to perform and transmit the ECGs. "We need people with the right education in the ambulance for these treatment networks to function properly," said Dr Schoos.

Dr Schoos continued: "Patients and their relatives should call the emergency telephone number (112 in Denmark and Turkey) at the first symptoms of a heart attack. Valuable time is lost when patients drive themselves to emergency rooms or are driven there by relatives and they could develop a bad heart rhythm on the way. Ambulance staff can do an ECG straightaway, even in the patient's home, and ensure that patients are taken to the right hospital immediately."

He added: "Calling emergency is also important in countries that don't use pre-hospital triage with telemedicine because ambulance staff can give anti-thrombotic medicine to dissolve or stop the development of a blood clot."

To improve treatment for patients who live more than 100km away from a hospital with balloon treatment, Dr Schoos recommended arranging the catchment area of a hospital into geographical zones. This would make it possible to identify patients who might have long treatment delays because they live further away.

Geographical zone 1 could be for patients who live within 100km of the balloon treatment centre. These patients can receive pre-hospital triage with telemedicine. Geographical zone 2 is for patients more than 100km away. These patients could be transferred by helicopter, or be treated first with clot dissolving drugs (thrombolysis) followed by balloon treatment when they arrive at the centre.

In the second study, Dr Burcu Demirkan from the Turkiye Yuksek Ihtisas Hospital in Ankara, Turkey, investigated the factors influencing the use of an ambulance among Turkish patients with an acute coronary syndrome (ACS).

"Until recently there was a limited ambulance service in Turkey but the Ministry of Health has now made it available for the whole country," said Dr Demirkan. "The service is free of charge and quicker than in the past. Despite these improvements most patients still prefer self transport instead of using an ambulance when they have acute chest pain."

For the study the researchers collected data from 330 patients with ACS from 2 hospitals (a state hospital without pPCI capability and an education and research hospital that could perform pPCI).

The study found that just 29% of patients went to hospital by ambulance. This was despite the fact that 68% of patients knew the emergency telephone number (112). There were no differences in age, gender and marital status between patients who called an ambulance and patients who did not.

Patients who called an ambulance had a higher level of education, greater knowledge of the risk factors for coronary artery disease, and greater awareness of the fact that chest pain is related to heart attack and patients should quickly seek medical care. These patients were also more likely to have ST-elevation myocardial infarction (STEMI) (as opposed to non-STEMI), vertigo, syncope/near syncope, nausea, vomiting and more severe chest pain.

Among patients who did not use an ambulance, 37% were given a lift, 14% drove their own car, 26% took a taxi, 12% used public transport and 11% walked to the hospital.

Most patients (whether they used an ambulance or did not) were accompanied by family members or friends during transportation to the hospital.

Dr Demirkan said: "Less than one third of Turkish ACS patients used an ambulance for transportation to the hospital even though this service is free. This rate was lower than rates from previous studies in European countries and Australia but similar to rates in China."

She added: "The perception of symptoms as not serious or not of cardiac origin was the most common reason for not calling an ambulance. However among the patients who considered the symptoms to be serious, most of them thought that self transport would be faster. Contrary to the assumption of these patients, we found that ambulance users arrived at the hospital more quickly."

Dr Demirkan continued: "Instead of using an ambulance most of our patients used transportation that was unsafe and inappropriate for ACS patients, who urgently need reperfusion therapy. This situation was due to patients' misperceptions about the symptoms of a heart attack or false assumptions that self transport would be quicker."

She concluded: "Our study improves understanding of the reasons why patients do not call an ambulance and will help with planning health education programmes to increase ambulance use. Such programmes should teach patients about the characteristics of chest pain related to heart attack and the importance of calling 112 when they experience these symptoms."

An elderly woman died alone after doctors failed to tell relatives they were ending her life on the controversial Liverpool Care Pathway. Olive Goom, 85, passed away with no one by her side after medics neglected to consult with her family about her treatment at Chelsea and Westminster Hospital. ?As Miss Goom lay dying alone, staff reassured relatives on the phone just hours before her death that there was no urgent need to visit ? even though doctors had already removed tubes providing vital food and fluids. Her family discovered that she had died only when her niece went to visit her and found she was already being prepared for the mortuary. They said last night that they will never be able to stop feeling guilty that no one was there in her final hours. The Mail has been contacted by several families who claim that relatives were put on the Liverpool Care Pathway ? the controversial system designed to ease the suffering of the dying in their final hours ? without any consultation. Some said they found out that their relatives were on the pathway only after they happened to read their medical notes; and by that time it was too late.

Keep in mind, by the way, that the Liverpool Care Pathway is sort of akin to the IPAB ?death panel? in Obamacare.

White House prepared to meet one-on-one with Iran

WASHINGTON (AP) ? The White House says it is prepared to talk one-on-one with Iran to find a diplomatic settlement to the impasse over Tehran's reported pursuit of nuclear weapons, but there's no agreement now to meet.

National Security Council spokesman Tommy Vietor said Saturday that President Barack Obama has made clear that he will prevent Iran from getting a nuclear weapon and will do whatever's necessary to prevent that from happening. Vietor said Iran must come in line with its obligations, or else faced increased pressure.

"The onus is on the Iranians to do so, otherwise they will continue to face crippling sanctions and increased pressure," Vietor said in a statement. He noted that efforts to get Iran back to the table with the five permanent members of the U.N. Security Council and Germany ? the so-called "P5+1" ? continue.

Iran has been a recurring issue in the presidential election campaign and Vietor's statement was released shortly after The New York Times reported Saturday that the U.S. and Iran have agreed in principle for the first time to negotiations. The paper said Iran has insisted the talks wait until after the Nov. 6 election.

Vietor, however, denied that any such agreement had been reached.

"It's not true that the United States and Iran have agreed to one-on-one talks or any meeting after the American elections," he said. We continue to work with the P5+1 on a diplomatic solution and have said from the outset that we that we would be prepared to meet bilaterally."

Obama and Republican presidential candidate Mitt Romney will meet Monday night in a debate focusing on foreign policy and Iran's nuclear ambitions will likely be a topic. Obama has said he'll prevent Iran from acquiring nuclear weapons. He hopes sanctions alongside negotiations can get Iran to halt uranium enrichment. But the strategy, which began during President George W. Bush's administration, hasn't worked yet. Obama holds out the threat of military action as a last resort. Romney has accused Obama of being weak on Iran and says the U.S. needs to present a greater military threat.

Despite unprecedented global penalties, Iran's nuclear program is advancing as it continues to defy international pressure, including four rounds of sanctions from the U.N. Security Council, to prove that its atomic intentions are peaceful.

Those sanctions, coupled with tough measures imposed by the United States and European nations are taking their toll, particularly on Iran's economy. Iranian authorities have in recent weeks been forced to quell protests over the plummeting value of the country's currency, the rial. The rial lost nearly 40 percent of its value against the U.S. dollar in a week in early October, but has since slightly rebounded.

U.S. officials say they are hopeful that pressure from the sanctions may be pushing Iran's leaders toward concessions, including direct talks with the United States. But several said on Saturday that they did not believe such discussions would happen any time soon.

If one-on-one talks are to occur, they would likely follow the model that the U.S. has used in six-nation nuclear disarmament talks with North Korea, the officials said.

In those discussions, U.S. negotiators have met separately with their North Korean counterparts but only as part of the larger effort, which also involves China, Japan, South Korea and Russia. Direct U.S.-North Korean talks are preceded and followed by intense consultations with the other members of the group.

However, the direct talks with North Korea have yet to bear fruit and U.S. officials warned that talks with Iran may not yield anything either. If U.S.-Iran talks do occur, they would likely be part of the P5+1 process, which groups the Britain, China, France, Germany, Russia and the United States and is overseen by the European Union. The group has met numerous times with Iranian officials but has yet to achieve any significant progress.

In late September, the group instructed EU foreign policy chief Catherine Ashton to reach out to Iran's top nuclear negotiator, Saeed Jalili, to organize another meeting. No date had been set for the possible resumption of talks.

Iran says its program is for peaceful energy and research purposes but Western nations fear the Islamic republic is determined to develop nuclear weapons and fundamentally reshape the balance of power in the Middle East. That would pose a grave threat to Israel.

Israel has threatened to strike Iran's nuclear facilities if Tehran doesn't stop uranium enrichment a process that can be a pathway to nuclear arms. Israel could decide to strike Iran's nuclear sites on its own, and Israeli leaders say time to act is running out. They have also hinted they would like U.S. support for any such attack.

An Israeli strike on Iran with or without Washington's involvement would likely draw retribution from Tehran including possible attacks on U.S. and Israeli interests overseas or disruptions to the transit of tankers through the Strait of Hormuz, which could send oil prices skyrocketing.

Obama has counseled patience as public as American public support for another Mideast conflict is low with the Iraq war over and the conflict in Afghanistan winding down.

Obama-Romney Debate a Chance to Delve Into Global Energy

While President Obama and Mitt Romney have done plenty of sparring over domestic energy-policy? issues?such as gasoline prices and who would drill more on public lands?they should both be prepared in Monday?s foreign-policy debate to lay out how they?ll take on the fundamentals of a rapidly changing world energy economy.

The global energy landscape confronting the next president is very different from the one that faced the candidates battling for the White House just four years ago?and it raises a host of complex new foreign-policy questions.

?You have to go back a long time to find a four-year period where the energy scene has changed so dramatically,? said Michael Levi, an energy-policy expert at the Council on Foreign Relations. ?We don?t just have shifting views on old policy debates, we have debates on issues that didn?t exist four years ago. Because of the political and economic situations, we?ve only been talking about energy domestically, but we haven?t scratched the surface internationally.?

Among the new international energy challenges facing the next president: Asia?s growing thirst for oil means that for the first time in history, developing economies such as China and India will consume more energy than the developed world?and compete aggressively with the U.S. for finite oil resources. The new explosion in U.S. natural-gas production offers opportunities for U.S. companies abroad?and raises questions as companies consider exporting the resource. Melting Arctic sea ice is opening up a new rush to drill for oil in the treacherous north?and possibly setting the stage for future environmental catastrophes. And underlying it all is the increasingly urgent problem of climate change caused by burning the coal, oil, and gas that fuel the global economy?but which scientists say could soon lead to catastrophic extreme weather, rising sea levels, and other disasters.

In a speech on Thursday on ?Energy Diplomacy in the Twenty-First Century,? Secretary of State Hillary Rodham Clinton spelled out many of those changes and said that her successor will need to place global energy policy at the heart of U.S. foreign policy.

?Energy will be one of the profound issues shaping the 21st century, and we are changing our foreign policy to reflect that,? said Clinton, who has said she expects to retire as the nation?s top diplomat no matter who wins the presidential election.

?Today, energy cuts across the entirety of U.S. foreign policy. It rests at the core of geopolitics. It?s an issue of wealth and power?. It is a matter of national security and global stability. It is at the heart of the global economy and is key to economic development. It?s even an issue of democracy and human rights,? she said.

While energy resources have long played a role in global conflict and diplomacy, their role is increasing and changing.

?This is a moment of profound change ... it raises complex questions about the direction we?re heading,? Clinton said.

Over the last year, Clinton has significantly elevated the role of energy as a driver of foreign policy. Last year, Clinton created the State Department?s Bureau of Energy Resources, charged with forging energy-driven foreign policy around the globe. In the coming weeks, she said, she will be sending out policy guidance to U.S. embassies to elevate reporting on energy and increase outreach to international energy partners.

Among the biggest changes: While the U.S. was the world?s biggest energy consumer and fossil-fuel polluter throughout the last century, China surpassed it in that role two years ago. Studies show that as China, India, and other developing nations continue to grow in the coming decades, the energy resources they choose could have a profound effect on the fate of the global economy?and the planet.

Growing Asian economies are fueled by an increasing appetite for oil, which could send demand and prices higher, dramatically increasing the pollution that causes climate change. It will be essential for the United States and its corporations, Clinton said, to be involved in driving the choices of new global energy consumers.

?The transformation to cleaner energy is central to the 21st-century economy,? she said. ?1.3 billion people don?t have access to energy?we have the technology and know-how that can help people leapfrog to energy that is reliable and affordable but also clean and efficient.?

Meanwhile, U.S. businesses are benefiting from a surge in domestic natural-gas production brought about by breakthroughs in ?fracking? technology. That boom could have a major impact on the world?s energy future, because natural gas has only about half the carbon emissions of coal. Today, U.S. companies are now working in China and Europe to help those countries develop shale gas, which could also help the U.S. achieve strategic geopolitical goals. Russia, the chief supplier of natural gas to Europe, has long wielded that resource as a power lever. But the State Department has created new initiatives for U.S. companies to help European countries develop their own natural-gas resources, which could limit Russia?s power. That?s good for the U.S., but it could also increase environmental concerns about the impact of fracking on water quality.

Another change to the global energy picture: Melting sea ice due to climate change has opened up the Arctic Ocean to oil drilling, and countries are rushing to be the first to exploit the resource. ?The Arctic is a frontier of oil and gas?and also a potential catastrophe. It?s critical we act now to set the rules of the road.? Clinton said. ?Four years ago, these issues didn?t have much currency, but now they?re seen as increasingly important.?

No matter who is the next president, the problem of climate change will need to be confronted. Many U.S. politicians, including Romney, have questioned whether oil and gas emissions contribute to climate change, but those views are out of step with the research of thousands of scientists across the globe. Meanwhile, more leaders in other nations are growing increasingly frustrated with the United States for failing to enact climate policy. Next month, at an annual United Nations summit on climate change, it?s expected that the U.S. will once again come under fire from the rest of the world for dragging its feet on the issue.

Although Obama and Romney have sparred repeatedly over domestic energy issues on the campaign trail, they have said little about how they would approach energy as a foreign-policy matter, let alone lay out how they would address climate change. But both of those issues will be at the heart of the nation?s foreign-policy future, and Monday?s debate provides an opportunity for some serious discussion.
